PlayNC master account name
Provide the account name that you chose when you created your PlayNC master account. If you don't remember your account name, please explain when submitting your information.

First and last name
Provide the first and last name you entered when you created your PlayNC master account. If you used an alias, please provide the alias that you used.

Address
Provide the postal mailing address (not e-mail address) that is listed on your PlayNC master account.

Date of birth
Provide the date of birth that you entered when you created your PlayNC master account. If you used a false date of birth, be sure to provide the date that you entered. If you are unable to provide this information, please explain when submitting your information.

Serial Codes/Access Keys
Provide all serial codes/access keys that have been added to your PlayNC master account. If you aren't sure where to find these,  click here. If you are unable to provide all of your serial codes/access keys, please explain when submitting your information.

Unique Account IDs
Provide the unique identifier for each game account listed on your PlayNC master account. A unique identifier was sent to you in an e-mail titled "Game Account Activated" on the day that you originally activated your game account. If you did not save your Unique Account ID somewhere, the only place to locate it is in that e-mail. You should submit one Unique Account ID for each game account that you have created on your PlayNC master account. If you are unable to locate your Unique Account IDs, please explain when submitting your information.
